This is a comprehensive outpatient diabetes education program that empowers individuals to take control in the successful management of their diabetes.

This intensive program includes:

√   Group sessions taught by a multidisciplinary staff **

√   An 8-week follow-up with a Certified Diabetes Care and Education Specialist

√   Individual appointments are available based upon provider’s recommendation

This program is either: Two Day sessions 8:30am-2:30pm   OR    Four Evening sessions 5:30pm-8:30 pm
